2022 USA Games Registration

All members of Team Indiana (athletes, Unified partners, coaches, and support staff) must complete several forms as part of the registration process for the 2022 Special Olympics USA Games. Before you download these forms and begin to fill them out, please take a moment to watch the applicable video below for detailed instructions.

• Form #1:  Athlete/Unified Partner Registration Form (Required)
• Form #2:  Athlete/Unified Partner Medical Form (Required)
• Form #3:  COVID-19 Supplemental Form (Required)
• Form #4:  Waiver for Communicable Disease (Required)
• Form #5:  Athlete/Unified Partner Release Form for General Liability (Required)
• Form #6:  Likeness Release for Sponsors (Optional)
• Form #7:  Disney Release Form for Adults (Required if age 18 or older)
• Form #8:  Disney Release Form for Minors (Required if age 17 or younger)
• Form #9:  Special Release for Atlanto-Axial Instability (AII) (Optional – submit if required by doctor’s exam)
